In consideration of my child being permitted to participate in the Columbus Day Soccer Clinic hosted by Jake Nerwinski and Brandon Valeri, I acknowledge that soccer is a physical activity involving inherent risks, including sprains, strains, fractures, concussions, and other injuries. I voluntarily assume these risks on behalf of my child.

I release and hold harmless the clinic hosts, coaches, staff, volunteers, and facility from claims arising from participation, except where prohibited by law. I authorize emergency medical care when I cannot be reached and accept responsibility for resulting costs.

I confirm the information provided is accurate, my child is physically able to participate, and I understand registration is not complete until payment is received.
